<p>Bad news for Craigslist users who like to peruse the Erotic Services Adult Services section of their site. It’s gone, replaced by a large black and white “censored” logo.</p>
<p>I’ve reached out to Craigslist for comment and await their reply. But the choice of words is significant – the section wasn’t simply removed, the censored word was used. </p>
<p>The site has been embattled as old press and state attorneys general use any excuse to blame sex crimes on the site. From South Carolina Attorney General Henry McMaster’s failed crusade against them to a variety of press stories about sex and other crimes. If it’s just a sex crime it isn’t a story. But if a listing on Craigslist was involved, it’s a big story.</p>
<p>Craigslist has fought back using little more than their blog and logic. And they’re right. Having prostitution up front and regulated, as Craigslist does, means less crime is associated with it. It’s not like prostitution, sometimes called the world’s oldest profession, was invented on the site.</p>
<p>The fact that eBay and others do exactly the same thing, but without human review and moderation, doesn’t seem to matter. Craigslist Sex is what scares the general population, and it’s what the press and the politicians will continue to use to get their hits and votes.</p>
<p>So the Craigslist Adult Section was removed. Is the world now a safer place? </p>
<p>Update: This only appears to affect U.S. sites, so if you’re looking for a happy ending in Saskatoon or the West Bank, have at it.</p>

<p>The BBC reports that researchers from Britain&#8217;s University of Kent gave children between four and 10 a variety of statements like, &#8220;this child is really clever&#8221; and &#8220;this child always finishes their work.&#8221; The kids were supposed to point to a picture of a boy or a girl depending on who they thought better matched the statement. Girls of all ages &#8220;said girls were cleverer, performed better, were more focused and were better behaved or more respectful,&#8221; while boys started out giving evenly divided answers, but as they got older came to agree with girls. Another study tested kids&#8217; math, reading, and writing skills — one group was told beforehand that boys usually did worse on the test, while the other was not. Unsurprisingly, boys in the first group did perform significantly worse.</p>
<p>Of course, no one wants boys to suffer self-esteem problems, either.  And that appears to be what this study is pointing to.  As Broadsheet points out:</p>
<p> This will hardly surprise anyone who has paid attention to the wealth of studies showing the devastating impact stereotypes can have on girls when it comes to math.  Even subtle reminders of gender &#8212; &#8220;male&#8221; or &#8220;female&#8221; check boxes, for example &#8212; can hurt girls&#8217; test scores, and of course the same is true for boys. Lead researcher Bonny Hartley explains: &#8220;There are signs that these expectations have the potential to become self-fulfilling in influencing children&#8217;s actual conduct and achievement.&#8221; She warns teachers to avoid pitting the boys against the girls and using diminishing sayings like &#8220;silly boys&#8221; or &#8220;schoolboy pranks.&#8221; </p>
<p>Still, I feel a little less concern over the boys, who thus far haven&#8217;t been subjected to a barrage of toys that tell them that their gender is less intelligent than the other.</p>
<p>The defining feature of this study is also proves that if your child is exposed repeatedly to idea that he or she is smart, that child will believe it.  It&#8217;s up to parents to build self-confidence, and for the rest of society to at least attempt not to chip away at it.</p>
<p>Growing smart, confident girls is our responsibility, and should be our pleasure as well.  And we can do it without cutting down boys, as well.</p>

in store. From Web behemoth Amazon to Brooklyn boutique store Shag,<br />
sales of an increasing range of sexual accessories are on the rise.</p>
<p>&#8220;Everyone says thank God you opened - it&#8217;s about time,&#8221; said Sam Bard,<br />
36, who co-founded Shag in Williamsburg last December.</p>
<p>&#8220;Each month&#8217;s been better than the last,&#8221; she said.</p>
<p>The &#8220;sexy shop&#8221; sells everything from vintage handcuffs to body paints<br />
and hosts sex-themed events like &#8220;introductory lessons in rope-bondage.&#8221;</p>
<p>Bard believes the recession helped the business, which opened on<br />
Roebling St. for rent considered low for the gentrified neighborhood.</p>
<p>&#8220;More couples are staying at home to save money, so rather [than]<br />
spending $150 on a one-time dinner, they will spend the same amount<br />
for toys that will continue to be used indefinitely,&#8221; she said.</p>
<p>Customers agree. &#8220;The expense of the toys is so insignificant compared<br />
to the growth of the relationship sexually,&#8221; said Heather, 40, who<br />
admits that even though she&#8217;s on a tight budget, she and her boyfriend<br />
laid down $150 on a &#8220;his and her&#8221; vibrator.</p>
<p>Sex accessories store Babeland, which has three outlets in the city,<br />
makes half of its sales online. It recorded an 18% spike in Web<br />
visitors and a 13% jump in sales in 2009.</p>
<p>&#8220;New Yorkers account for the biggest percentage of our online sales,&#8221;<br />
said spokeswoman Pamela Doan.</p>
<p>Meanwhile, even household company names are getting in on the action.<br />
Amazon launched its Sexual Wellness page in 2003 with 338 items,<br />
mainly condoms and lubricants. But it soon began to market more<br />
adventurous items - restraints, vibrators and flavored lubricants.</p>
<p>&#8220;We&#8217;ve seen this category grow pretty significantly over the last<br />
couple of years,&#8221; said spokeswoman Charmaine Diploa. Now the company<br />
stocks nearly 60,000 items, with adult toys and games being the<br />
biggest share.</p>
<p>A recent week&#8217;s best-selling item was the &#8220;Magic Wand Massager&#8221; - a<br />
nonphallic vibrator - and &#8220;Doc Johnson Lucid Dream #5&#8243; - a multispeed<br />
waterproof stimulator.</p>
<p>Amazon reckons its sales surge is due to its discretion - and unique<br />
brand of convenience.</p>
<p>&#8220;Customers receive an Amazon box and no one needs to know what&#8217;s<br />
inside except for that customer,&#8221; said Diploa. &#8220;And they love the fact<br />
they can purchase a vibrator, a watch and a Kindle all in one place.&#8221;</p>

<p>It has many names and many acronyms, SRS, GRS, GCS-Sex Reassignment<br />
Surgery. Whatever you call it, it is an irrevocable commitment and<br />
irreversible step should you choose to take it.</p>
<p>Male to female (MtF) transpeople talk about it a lot. You know: Are<br />
you, or aren&#8217;t you? Is she pre-op, or post-op? Who did yours? Whom are<br />
you going to have do yours? We talk about it a lot, except&#8230;when<br />
things go wrong. Then&#8211;we don&#8217;t say much at all. In fact, we won&#8217;t<br />
talk about it publicly, but it happens. We cover it up as if we should<br />
be ashamed. We feel damaged.</p>
<p>Something odd ensues, much like forty or fifty years ago. Back then,<br />
people spoke in hushed tones, if at all about the family member,<br />
colleague, or friend who had certain illnesses-the &#8220;C word&#8221;-only<br />
whispered. Many times this lack of openness about such matters led<br />
directly to preventable consequences&#8211;even deaths.</p>
<p>We need to talk about healthcare for the transgender individual and I<br />
want to talk&#8211;openly about my experience&#8211;about what can happen when<br />
things go wrong with SRS.</p>
<p>I Knew There Were Risks</p>
<p>While death is an unlikely result of complications from sex<br />
reassignment surgery, things can and do go wrong with the procedure.<br />
Even the most skilled of surgeons are, after all, human and as with<br />
any surgery, there are risks.</p>
<p>The information packet sent to me from my surgeon&#8217;s office listed the<br />
possible things that could go wrong. I had read extensively about what<br />
my body would have done to it after Propofol sent me to a dreamless<br />
sleep. Everything I found on the internet listed possible<br />
complications. One was a worst-case scenario, but the websites always<br />
mentioned it last and even then, it happened so seldom, most said,<br />
that it was a risk hardly worth mentioning.</p>
<p>My pre-operative appointment, a scant two hours before I signed the<br />
waiver, was brief. The surgeon asked a few questions, looked me over,<br />
pronounced me suitable for surgery and added that I had enough tissue<br />
for good depth. Almost as an afterthought, she said we needed to go<br />
over &#8220;this stuff&#8221;. We got to that last, not worth mentioning but still<br />
possible, worst-case scenario thing. Of course, it needed to be<br />
brought up but in almost four hundred surgeries &#8220;it had only happened<br />
just once, so we don&#8217;t worry about it&#8221;. I didn&#8217;t.</p>
<p>Just before I walked into the surgical suite, I signed the waiver.</p>
<p>I slumped to the floor of the shower that morning in Trinidad<br />
Colorado, water and reality raining down on me. Bewilderment and<br />
terror replaced hope, which swirled toward the drain along with the<br />
feces and the blood. Emergency surgeries there, four more at home in<br />
Kalamazoo and another in Denver have all failed.</p>
<p>Chronic pain, heavy narcotics addiction, and bouts of deep depression<br />
are the hushed legacies I have battled. Left with a possibly permanent<br />
colostomy and a painful, fibrous lump between my legs where a vagina<br />
should be, it is nearly impossible not to revisit the devastation<br />
daily. It is now two and a half years later.</p>
<p>The infrastructure of a natal male body is not the same as a natal<br />
female. The male pubic bone is lower and in my body, tipped somewhat,<br />
making it hard for my surgeon to get the geometry just right. In my<br />
case, there probably wasn&#8217;t really quite enough space to allow for the<br />
proper thickness of septum between the new vaginal wall and the rectal<br />
wall.</p>
<p>A slight tear in my colon from a retractor, was all that was necessary<br />
to compromise an otherwise flawless procedure. I had developed a<br />
rectal-vaginal fistula. This problem happens to natal women too, often<br />
as a complication from difficult childbirth or certain cancers.<br />
Frankly, the success rate for repairing the defect in natal women is<br />
not good either. It often takes multiple surgeries for them too. What<br />
makes it even harder for the transwoman?</p>
<p>The Endemic Problems In Transsexual Healthcare</p>
<p>My story&#8211;and my surgeon&#8217;s&#8211; illuminate important problems endemic to<br />
healthcare for the transgender and specifically, the transsexual person.</p>
<p>Lack of access- Discrimination by doctors and hospitals and inability<br />
to get insurance or pay outright makes it difficult, if not<br />
impossible, for many transgender people to access care - primary or<br />
specialized.<br />
Lack of knowledge- Documentation of protocols for care of patients<br />
undergoing transition is severely limited. Additionally, techniques<br />
for remedial care of complications are not well developed; instead,<br />
procedures designed for amelioration of fistulae in natal females are<br />
used.<br />
Lack of training- There exist no specialized courses of study or<br />
teaching hospitals for SRS surgeons.<br />
Lack of experience- While some surgeons learn techniques from other<br />
doctors already performing SRS, there are no residencies for those<br />
wishing to become SRS surgeons.<br />
Lack of inter-disciplinary collaboration- Ignorance, discrimination<br />
and arrogance on the part of specialists often block those needing<br />
specialized post-SRS care.<br />
In 2008 both the American Medical Association and the American<br />
Psychological Association issued white papers - policy statements<br />
calling for equal and fair access to healthcare for the transgender<br />
community. Both organizations recognized lack of access to health care<br />
as a growing crisis among transgender people.</p>
<p>Significantly, however, while both bodies asked the insurance industry<br />
to remove discriminatory blocks for transpeople, neither the APA nor<br />
the AMA asked for better training and documentation of skills. Neither<br />
organization called for unilateral coverage of transition related care<br />
and procedures. The APA resolution reads thus (emphasis mine):</p>
<p>THEREFORE, BE IT FURTHER RESOLVED THAT APA recognizes the efficacy,<br />
benefit and medical necessity of gender transition treatments for<br />
appropriately evaluated individuals and calls upon public and private<br />
insurers to cover these medically necessary treatments;</p>
<p>And the AMA:</p>
<p>RESOLVED, That our American Medical Association support public and<br />
private health insurance coverage for treatment of gender identity<br />
disorder as recommended by the patient&#8217;s physician.)</p>
<p>Ok, pretty strong statements, right? Look again and in particular,<br />
consider the italics. The APA resolution makes one BIG assumption-that<br />
you will be able to be &#8220;appropriately evaluated&#8221; , this requires you<br />
to have found and be able to pay for a psychologist, assuming of<br />
course, that you want to be evaluated. The AMA resolution assumes much<br />
the same thing; that you have a physician and that he or she is<br />
willing to make such recommendations, if, in fact, they are so<br />
qualified.</p>
<p>Whom Should I Go See When No One Will See Me?</p>
<p>I am associated with a LGBT resource center so, I get calls for things<br />
like, &#8220;Who is a trans-friendly electrologist&#8221; but the most frequent<br />
call I get is &#8220;My family doctor won&#8217;t prescribe me hormones. Whom<br />
should I see?&#8221;</p>
<p>To my knowledge, there are only two therapists in this area who take<br />
transgender clients, neither is a clinical psychologist who can render<br />
a surgical readiness opinion for SRS. There are one or two primary<br />
care physicians in the area who will prescribe hormones but, after<br />
hearing stories of one of them prescribing the exact same regimen for<br />
every patient, I am more than reluctant to send people to him. The<br />
nearest endocrinologist who takes transgender patients is ninety<br />
minutes from here. My own primary care doctor, admittedly, has no<br />
knowledge of how to dose a transwoman either pre-operatively or, post-<br />
operatively. He has left it up to me to tell him.</p>
<p>There is no curriculum, no specialization, no residency, and no board<br />
certification for SRS surgeons. No medical texts detail the<br />
procedures. Most SRS surgeons have followed in the footsteps of<br />
pioneers.</p>
<p>In my doctor&#8217;s case, the pioneer was legendary Colorado surgeon,<br />
Stanley Biber M.D. whose compassion for a social worker led him to<br />
develop the most celebrated sex reassignment practice in the States.<br />
The surgeon who did my SRS learned from him and took over his practice<br />
when he retired. Soon, she began refining his technique and developing<br />
and teaching her own. Today, she routinely performs SRS as many as two<br />
hundred times a year.</p>
<p>There are surgeons who want to do SRS-who have trained under doctors<br />
as mine did, but find it difficult, if not impossible, to find a<br />
facility that will grant privileges for SRS. Often, these surgeons may<br />
have the technical skills for SRS-urology, gynecological and plastic<br />
surgery, but hospitals will not grant access because there is no<br />
accredited course of specialization or board certification.</p>
<p>This may not be all bad. While there are a few new surgeons doing SRS<br />
every year, are they really getting the training necessary to<br />
accomplish not just the surgery itself but, adequate follow-up care<br />
too? Again, no curriculum, no specialized course of study, no board<br />
certification, but perhaps more significantly, certainly for the<br />
patient, there is no roadmap for what to do when things go wrong.</p>
<p>My Surgeon Doesn&#8217;t Know How to Fix It</p>
<p>Then, seemingly, neither does anyone else. My SRS surgeon attempted to<br />
fix the two-inch tear in my colon while I was still there, the repair<br />
failed in less than twelve hours. Next, a colon-rectal surgeon was<br />
found in the nearest large city and brought in to look at me. He<br />
aborted his attempt to repair the defect and instead, performed a full<br />
colostomy.</p>
<p>I flew home to Michigan with a bag attached to my abdomen and thick<br />
pads between my legs to soak up the blood.</p>
<p>A local colon-rectal surgeon, persuaded to see me by my primary care<br />
physician, was upbeat. Each time I went into surgery with him he would<br />
encourage me, saying &#8220;this time we&#8217;ll get it&#8221;. Coming out of surgery,<br />
my spouse Cindy would hear from him how well it went and that he was<br />
&#8220;optimistic this time&#8221; the closure would take and a new defect would<br />
not open up.</p>
<p>Another surgeon who has had some success with other fistula patients<br />
seemed promising, but eight months later, under the pressure of a<br />
barium test, that repair too, failed.</p>
<p>I have had stitch-overs, mucosal flap advancements, and pelvic floor<br />
muscle mobilization; each new approach diminishes what viable tissue I<br />
have for another attempt. I have tried to go into every surgery with a<br />
realistic outlook; multiple failures however, become difficult to bear.</p>
<p>Still, I am one of the fortunate ones with this complication; I stand<br />
a chance, albeit small, for an eventually favorable outcome. Many<br />
transwoman are not able to find a colon-rectal surgeon who is willing<br />
to work on a transperson, much less &#8220;someone else&#8217;s problem&#8221;, nor do<br />
all of us have the means.</p>
<p>Even if you are fortunate enough to locate a doctor&#8211;and in my case,<br />
privileged to have insurance and financial means to cover remedial<br />
procedures, then there is still only a slim chance for alleviation of<br />
this humiliating and debilitating condition. One surgeon, out of<br />
frustration no doubt, finally washed his hands of me saying, &#8220;well,<br />
you did this to yourself&#8221;.</p>
<p>Losing the Patient</p>
<p>I am not writing this to slam the physician who did my SRS. Far from<br />
it, she is a dear friend. We have shared much together, laughing, and<br />
holding each other; the two of us have grappled for footing. We have<br />
cried together, and yes, yelled at each other. She is an icon in the<br />
transgender community and heroine to many transsexuals. She is also<br />
perhaps the most accomplished of the few surgeons doing primary SRS in<br />
the United States.</p>
<p>However, even a surgeon as accomplished as mine cannot overcome<br />
obstacles that she has no training for and little experience<br />
resolving. There are unseen and unsaid barriers that come into play<br />
when someone with complications from SRS must seek treatment aside<br />
from their original surgeon.</p>
<p>As I mentioned before, most surgeons just do not want to deal with<br />
what they may perceive to be another doctor&#8217;s mistakes, if they are<br />
willing to look at you at all. A person would be hard pressed to get a<br />
doctor to admit that they are closed-minded, yet there are many<br />
doctors who simply will not work on-or treat a transsexual.</p>
<p>My surgeon has told me, and has repeated often, that she would see me<br />
through this. I believe she has tried, but somewhere in the mix of<br />
discrimination, ego, and fear, the patient has been lost.</strong></p>

<p>New Bodies, New Lives<br />
This spring, an Australian named Norrie May-Welby made headlines<br />
around the world as the world’s first legally genderless person when<br />
the New South Wales Registry of Births, Deaths and Marriages sent the<br />
Sydney resident a certificate containing neither M for male or F for<br />
female.</p>
<p>For a few days, it appeared that the 48-year-old activist and<br />
performer had won a long legal battle to be declared “sex not<br />
specified”—the only category that felt right to this immigrant from<br />
Scotland. May-Welby’s journey of gender identity can only be<br />
characterized as a long and winding road. Registered male at birth,<br />
May-Welby began taking female hormones at 23 and had sex-change<br />
surgery to become a woman, but now doesn’t take any hormones and<br />
identifies as genderless. The prized piece of paper May-Welby sought<br />
is called a Recognised Details Certificate, and it’s given to<br />
immigrants to Australia who want to record a sex change.</p>
<p>But the victory was short-lived. After so much publicity, it was<br />
perhaps inevitable that the New South Wales government would backtrack—<br />
which it did a few days later, saying the registry didn’t have the<br />
legal authority to issue a certificate with anything but male or<br />
female. May-Welby (who now goes by the single name Norrie) has filed<br />
an appeal with the Australian Human Rights Commission.</p>
<p>It’s easy to dismiss this case as just one more bizarre news story<br />
from Down Under, but May-Welby’s case could also represent the future<br />
of gender identity. Although no one is keeping statistics, researchers<br />
who study gender say a small but growing number of people (including<br />
some who have had sex-change operations) consider themselves “gender<br />
neutral” or “gender variant.” Their stories vary widely. Some find<br />
that even after surgery, they simply can’t ignore previous years of<br />
experience living as another gender. Others may feel that their gender<br />
identity is fluid. Still others are experimenting with where they feel<br />
most comfortable on what they see as a continuum of gender. “For some,<br />
it’s a form of protest because gender is such a strong organizing<br />
principle in our society,” says Walter Bockting, an associate<br />
professor and clinical psychologist at the University of Minnesota<br />
Medical School who has been studying transgender health since 1986.<br />
“Their identities expand our thinking about gender.”</p>
<p>In fact, some researchers compare the evolution in thinking about<br />
gender to the struggle that began a generation ago for gay and lesbian<br />
rights. Dr. Jack Drescher is a member of an American Psychiatric<br />
Association (APA) committee that is currently reviewing changes to the<br />
fifth edition of the Diagnostic and Statistical Manual, which is used<br />
around the world by clinicians, researchers, regulatory agencies, and<br />
insurance companies to classify mental disorders. DSM-5, as it’s<br />
called, won’t be published until 2013, but Drescher’s committee is<br />
reconsidering the diagnosis of gender-identity disorder, which<br />
encompasses people who do not identify with the gender assigned to<br />
them by biology.</p>
<p>The current debate echoes the controversy over the APA’s 1973 decision<br />
to modify the second edition of the DSM by declaring that<br />
homosexuality could be considered a mental disorder only if it was<br />
disturbing to the patient. Drescher’s committee thought about dropping<br />
the diagnosis of gender-identity disorder altogether, but realized<br />
that if it did, people who wanted treatment (sex-change surgery,<br />
hormones, or talk therapy) wouldn’t be able to get the diagnosis they<br />
need for insurance coverage. Instead, Drescher says, the committee is<br />
proposing changing the name to “gender incongruence” and making the<br />
diagnosis contingent on the person feeling significant distress over<br />
their gender confusion. “We didn’t want to pathologize all expressions<br />
of gender variance just because they were not common or made someone<br />
uncomfortable,” Drescher says.</p>
<p>But that seemingly simple change of language could help usher in a new<br />
era, in which a person’s gender could be expressed or experienced as<br />
male, female, “in between,” or “otherwise.” “People who work in this<br />
area have very flexible notions of gender,” Drescher says. “We don’t<br />
want to force people to fit into a doctor’s categories,” even though,<br />
he concedes, most cultures “tend to think in binaries.”</p>
<p>Bockting predicts that such binary thinking will eventually disappear.<br />
Many scientists, he says, see gender as a continuum and acknowledge<br />
that some people naturally fall in the middle. Gender, Bockting says,<br />
“develops between the biological and the environmental. You can’t<br />
always detect gender by physical evidence. You have to ask the person<br />
how they identify themselves; in that sense, it’s psychological.”</p>
<p>And gender isn’t synonymous with sex, he says, although the<br />
distinction may elude the layman. Sex, Bockting says, is assigned at<br />
birth based on the appearance of external genitalia. But, he says, “to<br />
determine a person’s gender identity, you have to wait until they grow<br />
up and can describe how they identify their gender.” And being<br />
genderless or gender-neutral isn’t the same thing as being asexual.<br />
“If you are asexual,” he says, “you are not interested in having sex<br />
with other people,” while gender-neutral people may be attracted to<br />
men, women, both sexes, or other people who are gender-neutral.</p>
<p>And while May-Welby’s story may seem out there, Bockting says it’s not<br />

doesn’t resolve all their gender-identity issues. “With time,” he<br />
says, “they accept a certain amount of ambiguity … We have this idea<br />
that people take hormones and undergo surgery and become the other<br />
gender. But in reality it’s more complicated.”</p>
<p>Even before the advent of sex-change surgery, there were always people<br />
who felt they didn’t fit into either gender. In India, a group of<br />
people called hijra have existed for centuries. They are typically<br />
biological males who dress as women but consider themselves to have no<br />
gender, Bockting says. There is also a long tradition of eunuch<br />
culture. Even today, other countries are more comfortable with the<br />
idea of gender variance. Drescher says that France has removed<br />
transsexuality from its list of psychiatric disorders and put it in<br />
the category of rare diseases. The British government has also<br />
declared that transsexuality is “not a mental illness,” but people who<br />
want a sex-change can get treatment under the National Health Service.</p>
<p>How all the debate will play out in this country is still unclear, but<br />
college students may be among those leading the charge for change.<br />
Many campuses—including Harvard, Penn and Michigan—now offer gender<br />
neutral housing and more unisex bathrooms to accommodate students who<br />
don’t fall neatly into male or female categories. The Common<br />
Application, which is used by most college applicants, just announced<br />
that it is considering adding voluntary questions that would give<br />
students a broader array of choices to describe their gender identity<br />
and allow them to state their sexual orientation, after gay advocates<br />
urged the change. How long before such changes begin to show up in<br />
other parts of society is unclear. But Drescher says he is certain of<br />
one thing after a lifetime of working with gender: &#8220;There is no way<br />
that six billion people can be categorized into two groups.&#8221; Now if we<br />
could only figure out the pronoun problem.</strong></p>
